HIFU
Focused ultrasound energy delivered to defined depths beneath the skin, where controlled heat prompts the tissue to contract and to build new collagen over the following months. No incisions, no downtime.

What is HIFU and how does it work?
HIFU stands for High-Intensity Focused Ultrasound. It is a non-surgical skin tightening procedure that uses sound waves rather than light, needles or scalpels. The device focuses ultrasound energy at a precise depth below the surface, in much the same way a lens focuses sunlight to a single point. At that focal point the tissue reaches a temperature high enough to create very small, controlled coagulation points, while the skin surface above is left intact.
Those micro-points are not damage in the everyday sense; they are a signal. The body responds by contracting the surrounding collagen fibres immediately and by starting a repair process — neocollagenesis — in which fibroblasts lay down fresh collagen and elastin over the weeks that follow. This is why the visible change from HIFU is gradual rather than instant.
The depth of the focal point is set by the cartridge fitted to the handpiece, and each depth targets a different layer:
- 4.5 mm reaches the SMAS layer — the fibromuscular sheet that a surgical facelift pulls on. This is the depth used for lifting along the jawline and cheeks.
- 3.0 mm works in the deep dermis and subcutaneous tissue, where laxity and volume loss show themselves.
- 1.5 mm stays in the superficial dermis and is used for finer skin texture and for delicate areas such as around the eyes.
Most treatment plans combine two or three depths in a single session so that the deeper supporting layer and the surface are addressed together. The ultrasound imaging built into many devices lets the practitioner see the layers before energy is delivered, which helps the energy land where it is intended.

When is HIFU preferred?
Mild to moderate laxity. HIFU is best suited to skin that has begun to lose firmness but still has reasonable elasticity. It is typically considered from the mid-thirties onwards, though the assessment is based on tissue quality rather than age alone.
Jawline and jowl definition. Softening of the lower face is one of the most common reasons people ask about the procedure, and it is the area where the 4.5 mm depth is most often used.
Neck and under-chin area. Loose folds under the chin and on the neck respond to the combination of immediate tissue contraction and later collagen remodelling.
Brow and eye area. Shallower depths can be used to address a heavy or low brow position and fine crepiness around the eyes.
Body areas. The upper arms, abdomen and inner thighs are sometimes treated for skin laxity, although body skin generally requires more energy and more patience than facial skin.
HIFU is not a substitute for surgery. Where laxity is advanced and there is significant excess skin, focused ultrasound will not produce the change a surgical procedure would, and an honest consultation should say so.
How does a session proceed?
Assessment. Skin thickness, degree of laxity, medical history, medication and any implanted devices are reviewed. The treatment areas and the depths to be used are decided at this stage.
Preparation. The face is cleansed of make-up and sunscreen, treatment lines are marked out, and ultrasound gel is applied so the energy couples properly with the skin.
Delivery. The handpiece is placed against the skin and energy is delivered line by line. Most people describe brief prickling or deep warmth as each line is applied — uncomfortable in places, particularly over bone, but generally tolerated without anaesthesia. A topical numbing cream or a mild oral analgesic can be arranged if you are sensitive.
Duration. A full face and neck session usually takes 45 to 90 minutes depending on how many areas and depths are included. Smaller single areas take less.
Afterwards. The gel is removed, a moisturiser and sunscreen are applied, and you can return to normal activity straight away. There is no dressing and no recovery period.
Who is it not suitable for?
HIFU has a favourable safety profile in appropriate candidates, but it is not for everyone. It is generally avoided in the following situations:
- Pregnancy and breastfeeding
- Active infection, open wounds or inflamed acne in the treatment area
- Implanted electronic devices such as pacemakers or defibrillators
- Metal implants, plates or prominent stents in the area to be treated
- Significant blood clotting disorders or ongoing anticoagulant use
- A history of keloid or hypertrophic scarring
- Uncontrolled diabetes, epilepsy or an active malignancy
- Recent dermal filler or thread placement in the same area — timing needs to be planned
- Severe skin laxity, where the expected benefit would not match expectations

When do results become visible?
There are two separate effects on two separate timelines. The immediate one comes from collagen fibres contracting under heat; some people notice a slight tightening within days, others notice nothing at first. This early effect is subtle and settles quickly.
The main change comes from new collagen, and that takes time to form and organise. Development usually becomes noticeable from around the second month and continues to build for roughly three to six months after the session. Progress depends on age, skin quality, smoking and general health, and it varies from person to person.
How long the result lasts depends on how quickly your own tissue continues to age. Many people choose to repeat the procedure after about a year to a year and a half. Sun protection, not smoking and a stable weight all help the outcome hold.
Temporary redness, mild swelling, tenderness to touch, small areas of numbness or tingling, and occasionally slight bruising can occur after a session. These usually resolve within days to a few weeks.
Combining HIFU with other procedures
HIFU addresses laxity and support; it does not restore lost volume or soften expression lines. For that reason it is often planned alongside other approaches within a wider non-surgical face lift plan — for example with botox procedures for dynamic lines, or with dermapen for surface texture. When several procedures are combined, spacing between them is decided during planning so each has room to work.
